Hospice is specialized
care provided for those with terminal illness.
Hospice care allows the client choices that
promote living fully, maintaining their dignity
and preparing for their transition gracefully,
with peace.
During this delicate period, we focus on supportive
care, so that you can focus on the quality
of your life. We tenderly support the client
and the family to live fully in the comfort
of home. |

| The Hospice Care Team of Hackley Visiting
Nurses recognizes that caring for a dying loved one
at home is one of the hardest tasks that a family will face
together. We recognize the physical and emotional challenges
that lay ahead. That is why we are here to support you and your
family through this very sensitive time |

| The VNS HOSPICE
TEAM is made up of several different Health Care Staff.
|
 |
A Registered Nurse who, in conjunction
with your physician and our Medical Director, assesses and works
aggressively to keep pain and other symptoms under control,
as well as offering emotional support. |
 |
Home Health Aides, for assistance
with personal care and grooming. |
 |
Homemaker services to assist with
errands and chores |
 |
A Social Worker and Spiritual
Advisor for counseling and emotional support as well as information
on available services in the community. |
 |
Volunteers for respite care, companionship,
and errands |
 |
Physical, Occupational, and Speech Therapy
as needed. |
 |
Dietary and other counseling as needed and
appropriate |
 |
Bereavement services are also provided to
the family for thirteen months following the death of the patient
so that they are supported during their time of grief. |

When should we contact VNS HOSPICE:
It is appropriate to initiate services when your physician feels
that further medical intervention will not offer a cure for
the disease, and that your life expectancy is six months or
less, if the disease runs its normal course. |

| What else should I know
about VNS HOSPICE? |
 |
Hospice does not provide 24 hour
care but will help you identify your support network. |
 |
Hospice pays for all medications
and equipment that are related to the terminal diagnosis and
as approved by the Hospice Care Team. . |
 |
Hospice care can be provided in
a Nursing Home or Assisted Living Center if the family is unable
to care for their loved one at home. |
 |
We can provide short term inpatient
respite services, in either Hackley Hospital, Lakeshore Hospital,
or a Nursing Home if the family is in need of a break from care
giving. |
 |
We will provide up to 16 hours per week of
respite care in the home. |
 |
We can authorize hospitalization if there
are problems with symptoms that we can not manage in the home
setting. |
 |
Hospice is a Medicare benefit and is also
covered by most insurance plans. |

| Our primary focus is the education and support
of the dying person and their family members. We understand
that in this final phase of life there are important jobs to
be finished and it is our goal to assist all in accomplishing
these tasks. We strive to support death with dignity, in accordance
with the wishes of the patient, in the environment of their
choosing, surrounded by those who mean the most to them. |
